Boise Advanced Therapeutics is following medical guidelines from the DOH, the CDC, as well as our national and local massage associations to ensure every one is as safe as possible. I believe it is essential for you to remain healthy. Massage can help keep your immune system strong and right now that is extremely important.

Rest assured you are and always have been in a clean and safe environment. The office is always sanitized with medical grade solution between each client, the table is dressed with clean, fresh linens and hand sanitizer is available upon arrival.

For added measures clients are asked COVID survey questions on intake form, in the reminder notifications (both email and text) and masks are required throughout your session per state regulations and our COVID-19 Policies

 

Masks are optional for clients and may request staff to wear a mask.
 

Ada County has rescinded the mask mandate and strongly recommends them. With so many clients being fully vaccinated & my high standards of cleaning protocols I am leaving mask wear up to you. If you prefer I wear a mask during your visit you may indicate it at the time of making your appointment or upon entering the building.

• I understand that COVID-19 is extremely contagious and may be contracted from various sources.

• I understand COVID-19 has a long incubation period during which carriers of the virus may not show symptoms and still be contagious.

• I understand that I am the decision-maker for my health care.

• To the best of their ability, my practitioner will provide me with information to assist me in making informed choices.

• Given the current limitations of COVID-19 virus testing, I understand determining who is infected with COVID-19 is exceptionally difficult.

• I understand that preventative measures and intensified sanitation protocols intended to reduce the spread of illnesses including COVID-19 have been implemented.

 

However, because this work involves close physical proximity over an extended period of time in a closed space, there may be an elevated risk of disease transmission, including but not limited to COVID-19.

• I hereby acknowledge and assume the risk of becoming infected with any illness including COVID-19 through this treatment and give my express permission to you and the staff at your offices to proceed with providing care.

• I knowingly and willingly consent to the treatment with the full understanding and disclosure of the risks associated with receiving care during the COVID-19 Pandemic.

• I confirm all of my questions were answered to my satisfaction.

• I have read or have had read to me, the above COVID-19 risk informed consent to treat.

• I appreciate that it is not possible to consider every possible complication to care

• I promise to be aware of and care for the health of the therapist working on me and have answered all questions in this form honestly.

• I agree with the current or future recommendation to receive care as is deemed appropriate for my circumstance.

• I intend this consent to cover the entire course from all providers in this office for my present condtition and any future condtion(s) for which I seek care from this office.

If you have any signs of being sick even if it is just a slight cold or allergies I ask you to reschedule your appointment ASAP per the Illness policies on the FAQ Policies page.

 

I am currently waiving the same day cancellation fee for illness related cancellation needs only. You may schedule your next appointment after 14 days of being symptom free. However the no show fee may still apply and is based on a case by case basis. To view our policies again Click Here.
